When you've finished telling us where the pothole is, click the Submit button (below) to email your report to the Highway Department.  Please use this form only to send information about potholes.  The only required part is the box labeled "Pothole Information", but if you'd like a reply please fill in the whole form.

Please tell us where the pothole is by giving an intersection or the street name and closest house number, and anything else you think will help.
